You've asked for a long post so here you get it.

The end of the UK? Maybe

Scotland:
Scotland had a vote for independence in 2014 which they said 'no' to, mainly because of the uncertainties about staying in the EU after independence.
Now however the UK (and not Scotland) wants to leave the EU and thus Scotland is pissed: they'll vote again and I assume they'll vote for independence (and stay in the EU).
Not very complicated there, now is it? The economy of Scotland prefers to stay in the EU because much of their economy means exporting to mainland EU (oil, gas, whisky, and textiles).
The EU subsidies have also proven to be a great help to Scotland because with these subsidies they've rebuild entire poor areas into much better areas.
That said: Scotland is PRO EU. I assume others may know more about the situation in Scotland but I think this is an alright description.

Wales:
They voted to leave the EU, just like England.
There's no reason to even assume they will wish to split from the UK.

Northern Ireland:
If you know the Irish history you'll know the situation in Northern Ireland is very difficult to solve.
Northern Ireland voted to remain in the EU, mainly because of their overland trade with Ireland.
They are in fact the only UK "country" with an overland border with an EU country (not counting the Canal tunnel).

Northern Ireland independent?
No, nobody wants that. Northern Ireland has a too small economy to become independent, they're relying on the EU, UK and Ireland too much for that.

Northern Ireland reuniting with Ireland?
About 50% of the population would like that to happen, but the other 50% doesn't.
50% of the population are Protestant, Unionists. The other 50% are Catholic, Republicans.
Unionists would like to stay in the UK, they are descendants of English and Scottish (Protestant) colonists.
The other 50% are Irish, Catholic Republicans, and wish to unite with the (Republic) of Ireland.
In reality I think less Northern Irish wish to unite with Ireland actually, because not all of the Catholics wish to unite, but not sure how the result of the EU vote changed that.

There is a small chance Northern Ireland will reunite with Ireland, the protestant population has been in decline (https://upload.wikimedia.org/wikipedia/commons/0/04/Ireland_Protestants_1861-2011.jpg).
It'd also mean having to get very strong arguments to get a vote in favour of a united Ireland. It'd mean having to convince a large number of moderate Unionists that it would be in their best interests.
No, not very likely to happen right now. Not with the past as it is (the Troubles, IRA, lots of fighting between Protestants and Catholics).
It is possible though. When a majority of the Northern Irish population wants to reunite with Ireland, so it will be according to the Good Friday Agreement.

The fact 55% of the population voted to stay in the EU mainly means some of the moderate unionists considered trade with Ireland too important.
Other probably voted brexit because well - English :p.

England:
